Faxes are not a joke in this context, they are the point. US healthcare referrals still arrive as faxed PDFs of scanned forms, which is precisely why this work resisted automation for two decades: the input is an image of a document filled in by hand by someone at another organisation. Reading that reliably is the hard part, and everything downstream depends on it.

Prior authorisation outcomes depend on payer rules that change without notice, a misclassified referral is a patient waiting rather than a failed API call, and a control tower shows you the queue without shortening it when the delay sits on the payer’s side.
